History
The cancer survivor population is expected to grow by 30 percent over the next decade. In 2013, co-founders Maddy Herzfeld and Carrie Tompkins Stricker, identified the need for cancer care providers to have the tools necessary to facilitate a better transition of patients from active treatment to life as cancer survivors. Thus, the evidenced-based, personalized and automated survivorship care plan was created.
Activating the patient’s voice
Carevive then expanded its care planning tools to help cancer patients and their families understand complex treatment concepts and to make informed treatment decisions aligned with their goals of care. With features such as geriatric assessments and shared decision-making built in the treatment planning tools, clinicians are now able to incorporate the patient’s voice systematically in the treatment planning process.
The importance of ePROs
Recent research emphasizes the importance of electronic patient-reported outcomes (ePROs) for proactive symptom management. We are passionate about integrating the patient experience into the care process, and our point-of-care and remote ePRO tools enable patients to report symptoms at any time. Our EHR-embedded platform enables providers to offer patients to receive real-time, evidence-based guidance to proactively manage side effects and avoid unnecessary ED visits and hospitalizations.
